一、假條的上方應該按照正式的格式寫上如下信息：

To：假條是遞給誰的 To: Peter Stone,

From：請假人 From: Lynn Chen, Financial Department Date：寫假條的日期(注意不是請假的日期) Date: April 2nd, 20xx

Subject：寫上請假字樣 Subject: Casual Leave of Absence

二、在您的請假信第一段，應該開門見山但是有禮貌地提出請假。第一段要中心明確，寫清您要請假的日期。

第二段範例如下：
